Dr. Sevinor will determine exactly where to administer several tiny injections to achieve the best results. No anesthesia is required. The entire procedure takes approximately ten minutes. It is unlikely you will develop significant side effects, although some people develop slight temporary bruising at the injection site, which can easily be covered by makeup. Botox Cosmetic relaxes the overactive muscles whose repeated contractions cause the lines between your brows to form. Botox Cosmetic is a natural, purified protein produced by the Clostridium botulinum bacterium. Very low doses of Botox Cosmetic are administered via a few tiny injections directly into the overactive muscles that cause lines to form. Botox Cosmetic works by blocking the transmission of nerve impulses to the injected muscles. The effects of Botox Cosmetic are usually confined to the injected region; it does not travel throughout the body.

Discomfort is minimal and brief. Once the injection is complete, there is usually no discomfort. Your lines gradually revert to their pre-treatment appearance if treatment is not continued.

Botox Cosmetic lasts up to four months.

Chin Contouring (Mentoplasty)

 

The basic procedure used involves the use of a chin implant. In this technique, the incision is made either inside the mouth or externally, under the chin. A plastic prosthesis, sized exactly to fit the patient, is placed in a pocket above the chin bone and beneath the muscles. Sutures are used to close the incision after which a pressure bandage is applied.

A mentoplasty can be performed under general anesthesia in which the patient is put to sleep, or, as more frequently done, under local anesthesia in which the area is numbed and the patient remains awake.

The best candidate for chin augmentation is the individual who has a receding chin and a normal dental bite. For those individuals with a bite dysfunction, jaw surgery, in conjunction with mentoplasty, may be indicated.

The patient is discharged from the hospital the same day. Pain or discomfort connected with the surgery is controlled with medication. On the day of the procedure, the patient is up and around, and, within a few days, is able to resume normal activities. The bandages are usually removed within a week following surgery.

Chemical Peel

 

The cosmetic skin peel will alleviate the crows feet around the eyes and the shriveled skin around the mouth and cheeks by removing the outer layers of skin. This method employs a solution that induces sloughing of the superficial layers of skin.

The patient is sufficiently medicated to avoid unnecessary discomfort with local anesthesia. A general anesthesia may be used if so desired.

The solution usually used in cosmetic skin peel is a phenol-croton oil mixture. It is applied with cotton swabs or a brush slowly and deliberately in order to prevent rapid absorption and deep penetration. If a deeper result is desired, waterproof tape is applied over the painted areas. This causes deeper coagulation and more pronounced skin sloughing when the tape is removed within one or two days. Shortly after the solution is applied, the skin becomes white and then turns red. The swelling begins to subside in two or three days. Meanwhile, a crust may form over the surface. This scabbing is necessary for the formation of new skin. When the scab comes off, smooth, pink, healthier, rejuvenated results are evident.

The amount of improvement is individual and depends on the initial condition of the patients skin. Most patients view the results as dramatic. Studies have shown that the processes long-lasting. Of course patients must be realistic as to expectations. This is an area of plastic surgery where it is very important for a plastic surgeon to have meaningful communication with the anesthetician. The patient must be instructed in an effective skin care regime and makeup technique.

The peel procedure can be done in a segmental fashion in specific areas of wrinkling or over the entire face. A partial peel can also be done in conjunction with a face-lift.

The procedure lasts from 20 to 90 minutes depending upon whether it is a partial or full face peel. For a day or two following the procedure, the skin will feel as if it has been sunburned, but medications can alleviate any discomfort. Swelling of the affected area is to be expected for about ten days following the procedure.

Cosmetic Eye Surgery

Cosmetic eye surgery is designed to improve the appearance of the individual by removing the excess fat, and wrinkled, droopy skin of the upper eyelids that can make you look tired and sad and eliminating the “bags” under the eyes. The patient must be thoroughly aware that each and every wrinkle and fold of the skin around the eyes cannot be removed. The results of aesthetic eyelid surgery are long-lasting. Removal of fat is permanent and generally will not recur, except rare occasions. Patients should be advised that it is impossible to estimate or guarantee any definite length of time the effects of surgery will last due to variation of skin type, wrinkles, age and the healing ability of the individual. One must accept the judgement of your plastic surgeon and realize that he will remove all the skin that can safely be removed to effect the most suitable result for your particular problem. It will be necessary for Dr. Sevinor to know if you were ever told by an Opthamologist that you have a condition called “dry eye” or if you ever have any other eye problems.

Surgery is performed as a hospital or clinic procedure. DO NOT WEAR ANY MAKE -UP, FINGERNAIL POLISH OR FALSE EYELASHES ON THE DAY OF THE SURGERY. Surgical time is variable; approximately thirty minutes for each eyelid. Cosmetic Eye Surgery is usually done under local anesthesia, with additional sedation to relax the patient. Medication is given just prior to the operation by vein, to help produce amnesia while the surgery is being performed.

The incision for reducing the wrinkles and excess skin of the upperlids is made horizontally within the natural fold of the upper eyelids and extends slightly beyond the outside corner into the existing creases. A segment of skin is removed and sometimes portions of fat underneath the lids are removed. The incisions for the lower lids are made of approximately 1/16th of an inch below the rim of the lower eyelid and extends beyond the edge of the eye. A segment of skin and underlying fat are removed. These incisions are well camouflaged by a natural crease. In some cases and incision is made inside the lower eyelid to remove only excess fat.
The scars are permanent, however, they are placed in such a position that they are barely discernible after a reasonable period of time.
The eyelids may be discolored and swollen for a period of time following the surgery, depending upon the type of skin and healing ability. Occasionally the white portion of the eye may be “bloodshot”. This is a temporary condition.

After surgery, 4 x 4 gauze sponges moistened in a basin of ice water may be applied intermittently over the lids for several days. This helps alleviate swelling and usually gives soothing affect to the operative sites. Dark glasses are suggested for a period of one week, to prevent irritation from sun and wind and rest the eyes, even when indoors. The patient should remain within reasonable traveling distance of the doctor’s office for approximately one week. Sutures are usually removed during the first week. Make-up can be applied approximately 10 days following surgery. All coiffure procedures can be carried out approximately ten days after surgery.

Ear Reshaping (Otoplasty)

 

No pair of ears are exactly the same either before or after the operation; for example, the ear on the left side of the head may be larger and further away from the head than the ear on the opposite side. Convolution of the ears is always irregular in ears that have undergone such an operation. This is emphasized because the patient’s parents and friends are more critical of a person’s ears following surgery than they were before surgery. The surgeon makes every effort to improve the appearance of the ears so that they will be satisfactory from a cosmetic stand point. In rare instances, at the discretion of your surgeon, a secondary minor procedure may be performed to modify the final result. It is not infrequent that the surgeon sets the ears in quite closely to the head since nature tends to “unspring” the cartilage somewhat in the initial months following surgery and this over-correction is performed to create the ideal position several months following surgery.

Incisions are made behind the ear. The cartilage is exposed and the operative procedures are carried out to reform the cartilage framework of the ears, so that they may be placed in a more normal position. Stitches are placed in the cartilage to hold the ears in place. The skin is stitched in routine fashion.

The operation may be performed in the hospital or clinic under general anesthesia, at the surgeon’s discretion. If performed as a clinic procedure, the patient returns home following surgery. Hospital stay for children, under general anesthesia, is approximately 1 day. For adults and children, under local anesthesia, as outpatient or hospital stay of approximately one day.
Surgical time varies: approximately one hour each ear.

Immediately after the surgery, bandages are placed over the ears and around the head. The bandage is left on for approximately three days. Following the removal of the bandages, the patient should wear and elastic bandana to cover the ears while sleeping, an additional three to four weeks. The patient undergoing such ear surgery should expect some soreness the first day or two which will be alleviated by medication.

Take all medications exactly as ordered.
You my be requested to take some medicines before your surgery. DO NOT TAKE ASPIRIN OR ANY ASPIRIN-CONTAINING COMPOUNDS FOR TWO WEEKS BEFORE OR TWO WEEKS AFTER YOUR SURGERY. Acetaminophen (Tylenol, Valadol, Tempra) may be used in place of aspirin. AT THE FIRST SIGN OF HIVES OR A RASH, DISCONTINUE ALL MEDICATION AND CALL IMMEDIATELY.

Possicle complications are occasional minor bleeding or discharge from the incision areas. Infection and/or wide scars are possible but uncommon. If excessive pain, call your surgeon.

Face Lift (Rhytidectomy)

 

This is considered a major operative procedure. Like any extensive surgical procedure, operative incisions and complicated surgical stitching are required. Contrary to some non-medical reports, it is not possible to remove all the folds and eliminate all the wrinkles of the face, regardless of the treatment applied. You must accept the judgement of the plastic surgeon and realize that he will only remove the skin of the face that can be done safely and suitable for your particular facial contour. The face lift procedure is directed toward improvement and can not in any way guarantee that each and every wrinkle and fold of the skin will be removed. The type of skin and age of the patient are important factors in the final result.

The most common question in regard to a face lift is ‘how long will it last’? This is impossible to state, as there are many factors involved – the amount of skin removed; age and general condition of the patient; type of skin as well as it’s condition and texture; amount of previous sun and wind exposure as well as healing ability. Skin aging continues after this operation, even though the excess skin and wrinkles have been removed. On rare occasions, a minor secondary procedure may be advisable. The operation does not stop the aging process.

The operation is performed in the hospital and the average stay is two days. The patient is admitted to the hospital the morning of surgery.

The incisions are within the temple area, in front of the ear, behind the ear and back into the scalp over the mastoid areas. After the incisions are made, the skin is pulled in the proper lines of tension and the excess removed. Sutures are placed in deeper structures to assist maintaining stability. The incisions are then closed with surgical stitches.

Lip Enhancement (Luscious Lips)Dr. Sevinor was the first surgeon to demonstrate lip enhancement on National television on the Oprah Winfrey Show.

 

For centuries, a woman’s lips have been viewed as a sign of her sensuality. Look around. It seems that luscious lips are everywhere. Luscious lips are a key element of almost every photo in every fashion magazine. Unfortunately, not everyone is blessed with a pair of naturally beautiful lips. The few who are don’t get to keep them without a little help. Over time the sun, the elements, smoking and the aging process take their toll. The corners of the mouth begin to droop. The V-shaped area of the upper lip known as Cupid’s Bow begins to flatten out and lose its youthful fullness. Vertical wrinkles or Smoker’s Lines begin to appear, causing your lipstick to bleed. That once sensual look is gone or greatly diminished.

Thanks to today’s many medical advances there is help. Aging lips can be rejuvenated and less full lips can be enhanced. Options include lip enhancement with injectable Zyplast collagen or permanent enhancement with a lip implant.
The injectable technique utilizes Zyplast collagen. Artecoll, HylaForm and Restylane are options that may be available soon. Zyderm and Zyplast injectable collagen are used to enchance the natural silouette of the upper border of the upper lip and Cupid’s Bow. They also fill in the vertical lines along the upper lip that often cause lipstick to “bleed” beyond the lip line.

SoftForm and soft “ePTFE” (expanded polytetra-fluoroethylene) Gortex implants lead the way in synthetic options. SoftForm and soft “ePTFE” implants have proven to provide a safe, predictable, non-allergenic, permanent improvement for thin or aging lips.

Following lip enhancement with one of the injectable products, your lips remain
swollen for a few hours only. You may return to normal activities the same day. Touch-up treatments may be necessary.

When implants are used for lip augmentation, discomfort is minimal. Small stitches may be used to close the implant site. These stitches will be removed in a few days. Some swelling may last up to a week. Elevating your head and applying cold compresses during the first 24 hours will help keep this to a minimum. Postoperative discomfort can be controlled with medication. Chewing food and brushing your teeth may be a little uncomfortable for a few days. There may be some bruising which can be covered with make-up.

Neck Sculpting (Submental Liepctomy)

Many men and women have what they describe as a “double chin”; a localized area of fat below the chin. This is usually hereditary, and no matter how much one attempts to diet and exercise, it is unlikely it will improve. Liposuction of this area can leave irregularities in the contours of the neck which appear as lumps and bumps.

Instead of liposuction, Dr. Sevinor makes a one-inch incision in the natural crease under the chin, and he separates the skin from the fat. He then lights up the neck with a fiber-optic light, and under direct vision is able to remove the excess fat, and if necessary, tighten the muscle to give a more sculpted, youthful, and thinner appearing neck.

This procedure takes 1 to 1 1/2 hours and can be done under local anesthesia with sedation. It is not painful and downtime is 5-7 days. Dr. Sevinor has performed this procedure on men and women ages 20 to 80.

Nose Reshaping (Rhinoplasty)

 

The operation is done in the hospital and requires minimal hospitalizations or under certain circumstances, may be performed on an out-patient basis after which the patient returns home. Usually Dr. Sevinor’s patient’s remain in the hospital overnight. Usually this is performed under general anesthesia. However this surgery may be performed under local anesthesia, plus sedation and intravenous medication just prior to surgery,
to help the patient be completely relaxed and help produce an amnesic state for the surgical procedure. The operation itself should not be painful.

The incisions for reducing or recontouring the external nose are made inside the nose, or sometimes small incisions are made on the rim or the base of the nose. The nasal skin is lifted away from the cartilage and bone and the excess tissue is removed. The cartilage of the tip of the nose may be changed if necessary. Frequently, the nose is shortened to adapt to the new contour. Such surgical procedures are necessary to reform the nose to the desired contour. The nasal bones are usually narrowed. The excessive skin left by the reduction of the underlying bone and cartilage is elastic and readapts itself to the new framework in most instances.

The operative procedure and the postoperative period are not exceedingly painful. Since there will be packing in the nose for several days following the surgery, the patient will breathe through his mouth. This will give the feeling of having a minor cold and may cause drying of the membranes of the mouth. The nose may feel “congested” for several weeks following this surgery due to internal swelling.

The eyelids may be discolored and swollen for a period of seven to fourteen days, depending upon the type of skin of the patient, age, healing ability and the amount of surgical correction necessary. Occasionally, the whites of the eyes will be bloodshot and the upper lip may feel “stiff” and unnatural for a varied period of time. Some numbness may be noted over the tip of the nose for several weeks to months.

Frequently the patient requesting cosmetic surgery of the nose has another condition which causes difficulty in breathing. The operation for correction of this can be carried out at the same time as the rhinoplasty. (This portion of the operation as well as part of the hospital and operating room fees, may be covered by an existing insurance policy. Discuss this aspect of your surgery with your insurance agent prior to your surgery to avoid any dispute between you and your insurance carrier following the surgery.)

The cosmetic procedure of reconstructing the nose is one of the most popular operations performed by plastic surgeons today. The patient must accept the judgment of the plastic surgeon as to the type, shape, and contour of the nose as he is better acquainted with facial contour and what will suit the face best. Under no circumstances can the patient “select” a nose. Such a selection by the inexperienced could result in a mismatch for his particular face.Rhytidectomy is the removal of lax skin, or wrinkles. Wrinkles can be related to age, heredity or environmental factors. Not everyone ages at the same rate. A great deal of the aging process relates to your tissues and what you do to care for them.This procedure intended to place protruding ears in a more normal position. Protruding ears in a child can create a psychological problem which may impede his social and emotional development through teasing by his peers. The operation my be performed any time after age four. It may be desirable to have the surgery performed prior to beginning elementary school to avoid the danger of psychotrauma from teasing classmates.This non-surgical procedure is used in the treatment of fine facial wrinkling, hyperpigmentation abnormalities of the face, and several other facial skin problems. This technique has been performed by plastic surgeons in the United States since 1960 as a safe and effective method to improve facial wrinkles which are the result of aging, sun damage, and hereditary factors.Chin contouring has helped thousands of people improve their appearance by altering the contours of their faces. A chin augmentation is designed to add size to a receding chin. In some cases, a chin augmentation is done in conjunction with lipo-suction surgery and other cosmetic facial procedures such as rhinoplasty (surgery of the nose) or rhytidectomy (facelift).
